This review explores the molecular targets implicated in neurodegeneration, highlighting the role of misfolded proteins, oxidative stress, and neuroinflammation. Recent advances in drug discovery, including small molecules, biologics, and nanomedicine, are examined, focusing on their mechanisms of action and clinical efficacy. The potential of structure-based drug design, high-throughput screening, and the integration of artificial intelligence in identifying novel compounds is also discussed. Furthermore, the challenges of blood-brain barrier penetration and the need for personalized medicine approaches are emphasized. By synthesizing current knowledge and innovative strategies in medicinal chemistry, this work aims to contribute to the development of targeted therapies that can slow or halt the progression of neurodegenerative diseases. Keyphrases: BET, Bromodomain, Epigenetics, inhibitor, kinase
